Position Summary Pathway of Hope is a new initiative within the California South Division, which aims to break the cycle of generational poverty within families. The Case Manager will provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of client needs. The Case Manager will assess the housing, health, financial, and psychological needs of program participants, as well as collaborate with the Salvation Army Corps and community partners for a holistic and wrap around approach. The Case Manager will develop individualized action plans to meet identified goals that work towards moving the client from crisis to self-sufficiency by increasing hope. This position is accountable to the County Coordinator/Supervisor. The overall goal of the position is to enhance the quality of client management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management through Strengths Perspective and Motivational Interviewing. The Case Manager will travel between sites in the county and work with staff to build capacity and sustainability of Pathway of Hope. Essential Functions 1. Conduct intake assessments with new clients. 2. Participates in all trainings for Pathway of Hope & WellSky Community Services; including but not limited to Strengths Based Perspective, Motivational Interviewing, Trauma Informed Care/De-Escalation, and Stages of Change Model3. Gain and maintain a comprehensive knowledge of WellSky Community Services.4. Complete all data entry requirements for WellSky Community Services.5. Development of an individualized action plan and set of goals for each client. 6. Provide on-going case management/assessments for clients at 30, 60, 90 and 12 months. 7. Provide referrals to clients on housing, health benefits, financial support, and available community and public services. 8. Maintain a current file of appropriate community resources for staff use. 9. Network with outside appropriate agencies and build relationships with community partners.10. Compile monthly statistical reports as required by the Division. 11. Attend monthly case management meetings to update the team concerning service plan progress and emerging needs.12. Maintain positive working relations with Salvation Army representatives, volunteers, and all agencies providing services to clients. 13. Travel between sites and work closely with Social Services staff at the Corps on all Pathway of Hope cases.14. Participate in meetings with the Divisional Pathway of Hope Coordinator15. Other duties as assigned by the management team. Working Conditions Must be able to sit, walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Must be able to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Must be able to lift up to 25 lbs. Minimum Qualifications 1. Bachelor of Arts in Social Work preferred or related field and/or minimum of 2-3 years of Case Management experience. 2. Must possess a Valid Class C California Drivers License.3. Minimum of one-year experience working with low-income and/or homeless individuals and families. 4. Must be able to pass a criminal background check.5. If working in vicinity of children a criminal background check is required with certification for Protect the Mission policies and procedures. Skills, Knowledge & Abilities 1. Knowledge of the unique issues of homelessness. 2. Ability to work with diverse populations in a caring, sensitive, and non-judgmental manner.3. Basic understanding of mental health disorders4. Knowledge of Strengths Perspective, Stages of Change Model, and Motivational Interviewing5. Ability to read, write, speak understand English. 6. Bilingual English/Spanish is required.7. Proficient in Microsoft Windows and use of Word and Excel.
