Job Description:
Description Position Summary Provide interpretation and
translation services, culturally appropriate health education and
information, informal counseling and guidance on health behaviors,
and advocate for individual and community health needs. This
position includes Client Orientation to Comprehensive Peri-natal
Services Program (CPSP) initial assessment, trimester
reassessments, postpartum assessment, interventions, and follow-up
services in: Obstetrics, Nutrition, Health Education, Psychosocial
and Individual Case Coordination. Schedule Flexible, may require
some evening and weekends Primary Duties and Responsibilities
Orient the patient to comprehensive perinatal care at entry of care
and throughout the pregnancy as needed Complete assessments in the
four components – obstetric, nutrition, health education and
psychosocial using approved assessment forms Complete the
Individualized Care Plan (ICP) with the patient following the
initial assessments. Prioritize problems and actions planned to
resolve them Provide individual Perinatal Education, counseling,
referrals and instructions as appropriate to the needs and risks
identified on the ICP Reassess in each trimester to identify
changes or new developments since the previous assessment and to
provide continued support for the patient’s strengths. Revise and
update the ICP as needed Postpartum Assessment and Care Plan Assess
the mother and infant postpartum and update the ICP. Provide
appropriate interventions for the patient and her baby Track
patient’s attendance at appointments, and identify the reason for
any missed appointments Responsible for issuance of Presumptive
Eligibility if patient is qualified for the Program. Maintains
OB/EDC log, Newborn log and sends monthly report to Managed Care,
QA Director and Clinic Supervisor. Ensure that appropriate copies
of the prenatal record are at the hospital during the Intrapartum
period (34 weeks gestational age) Assist the patient to obtain
Medi-Cal and explain what documents she must have to apply for
Medi-Cal. Inform Supervisor of any issues or problems Time Studies
Maintain a positive working relationship with staff Other duties as
designated by the Other duties as designated by the Clinic Manager,
Clinic Director, leading Medical Provider, or Clinical Service
Director Qualifications Minimum Requirements • Microsoft Office
(Word, Excel, Outlook) • Excellent Communication Skills • Excellent
Customer Service • Bilingual English/Spanish preferred • Ability to
work as a team player and work independently • Reliable
transportation • Must be able to travel from facility to facility •
This position requires 10 % travel outside the local area. Required
Education/Experience and/or Licensure/Certification s • Medical
Assistant Certification or higher • High School Diploma and/or GED
equivalent required. • Certified Comprehensive Perinatal Health
Worker • At least one year of full-time paid clinical perinatal
experience and must be certified CPHW • Participate in ongoing
trainings and continued education for skills development •
Commitment to goals and philosophy of Northeast Community Clinic •
Current CPR and/or BLS Certification • Valid State Identification
Physical Requirements and Working Conditions • OSHA Category 2 –
Involves no regular exposure to blood, body fluids, or tissues, but
may require unplanned tasks that involve exposure to blood, body
fluids, or tissues. • The work is majority of the time
non-sedentary in nature. • The employee is regularly required to
communicate, frequently required to use repetitive motions, move,
remain stationary, regularly push, pull and lift up to 20 pounds
and occasionally push, pull and lift up to 40 pounds. • Must
possess mobility to work in a standard office/clinic setting (in
some cases both settings) and to use standard office/clinic
equipment, including a computer, stamina to maintain attention to
detail despite interruptions, read printed materials and use a
computer, and communicate in person and over the telephone.
