Job Description:
About the Role: The BHU Case Manager plays a crucial role in the
health care services sector by providing comprehensive case
management for patients within the Behavioral Health Unit. This
position is responsible for assessing patient needs, developing
individualized care plans, and coordinating services to ensure
optimal patient outcomes. The Case Manager will work closely with a
multidisciplinary team to facilitate communication and
collaboration among health care providers, patients, and their
families. By advocating for patients and navigating complex health
care systems, the Case Manager aims to enhance the quality of care
and support recovery. Ultimately, this role is vital in promoting
mental health and well-being within the community served by Mission
Community Hospital. Minimum Qualifications: Current licensure or
certification in the state of California as a LVN nurse At least
two years of experience in case management or a related field
within health care services. Preferred Qualifications: Experience
working in a behavioral health setting or with diverse populations.
Familiarity with electronic health record (EHR) systems and case
management software. Responsibilities: Conduct thorough assessments
of patients' mental health and social needs to develop tailored
care plans. Coordinate and facilitate access to necessary services,
including therapy, medication management, and community resources.
Monitor patient progress and adjust care plans as needed, ensuring
that all interventions are effective and appropriate. Collaborate
with a multidisciplinary team, including psychiatrists, social
workers, and nursing staff, to provide holistic care. Maintain
accurate and up-to-date documentation of patient interactions,
treatment plans, and progress notes in compliance with health care
regulations. Skills: The required skills for this position include
strong assessment and analytical abilities, which are essential for
evaluating patient needs and developing effective care plans.
Excellent communication skills are vital for collaborating with
