About the Role: The BHU Case Manager plays a crucial role in the health care services sector by providing comprehensive case management for patients within the Behavioral Health Unit. Minimum Qualifications: Current licensure or certification in the state of California as a LVN nurse At least two years of experience in case management or a related field within health care services. Preferred Qualifications: Experience working in a behavioral health setting or with diverse populations. Familiarity with electronic health record (EHR) systems and case management software. Responsibilities: Conduct thorough assessments of patients' mental health and social needs to develop tailored care plans. Coordinate and facilitate access to necessary services, including therapy, medication management, and community resources. Monitor patient progress and adjust care plans as needed, ensuring that all interventions are effective and appropriate. Collaborate with a multidisciplinary team, including psychiatrists, social workers, and nursing staff, to provide holistic care. Maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress notes in compliance with health care regulations.
